Patient's Rights & Responsibilities
We at The Far Eastern Memorial Hospital view health care as a partnership between you and ourselves, your health care providers. We respect your rights, values, and dignity. We also request that you recognize the responsibilities of being a patient.
Both the patients and health care providers should understand these rights and responsibilities in order to facilitate good communication, cooperation and formation of trust between them. Please contact the hospital’s staff if you have any questions or require advice regarding these rights and responsibilities.

Patient's Rights

  • You have the right to receive safe and high quality medical care without discrimination.
  • You have the right to know the condition of your health, including your diagnosis, treatment plan, alternative treatment and likely results, risk associated with your recuperation, and to engage in consultations regarding this with your doctor.
  • You have the right to make decisions about the plans of care prior to and during the course of treatment (including tests, treatment and operation). You have the right to refuse a recommended treatment or plan of care to the extent permitted by law and hospital policy. In the case of such a refusal, you have the right to be informed of the medical consequences of such action. You are entitled to receive any other appropriate care and services that the hospital provides or the right to transfer to another hospital.
  • If your treatment requires you to have any operations or invasive medical procedures, we are obligated to provide you and your family with a full breakdown of the relevant risks and success rates. We will not perform operations without informed consent unless urgent intervention is required and such intervention is permitted by law.
  • We have a duty to protect your privacy to the extent required by law. We may appropriately disclose the patient’s condition to his/her family members upon request. If you wish to withhold such information from a particular family member, a written request must be submitted to the nurse station or the attending physician.
  • Since the Far Eastern Memorial Hospital is a teaching hospital, you are strongly encouraged to cooperate in teaching activities for the promotion of medical education, and the cultivation of qualified in house medical personnel. However, you have the right to refuse laboratory exams, tests, or any activity unrelated to your treatment. Such refusal will not have any impact on the attitude of the hospital staff towards a patient or the quality of care they provide.
  • You have the right to request copies of examination reports, certificates of diagnosis, or summaries of medical records on payment of a reasonable fee.
  • In accordance to the patient's right to autonomy, you have the right to choose the Hospice Palliative Care Act. The "Preceding Declaration of the Authorized client to grant medical decision making" can be obtained from the nursing station. By filling out this form, you can appoint a person to act on your behalf in the event that you are unable to express your wishes.
  • You have the right to complete the treatment course by receiving continuous medical care and follow-up which will not be suspended or interrupted with no reason.

Patient's Responsibilities

  • You are responsible for providing accurate and complete information about present complaints, past illnesses, hospitalizations, medications and other matters in relation to your health, such as medical history, history of allergies, etc. Detailed medical information will assist medical staff in providing the appropriate course of treatment.
  • You must ask your doctor, nurse practitioner, head nurse, or other appropriate personnel if you do not understand your diagnosis, treatment or prognosis.
  • You are responsible for following the care plan that is provided for you. If you are unable or unwilling to follow the care plan, it is your responsibility to inform your health care provider. Your health care provider will explain the medical consequences of not following any recommended treatment. You are responsible for the consequences of not following your care plan.
  • You are obligated to follow the facility’s rules and regulations. You must respect the medical staff and also the privacy of other patients.
